GENERAL DIRECTIONS AND MAP:

From I-25: From I-25 take the Orchard Road exit and go west (towards the mountains). Take your first left (turn south) into the driveway for the Orchard Falls building which will then be on your right. Parking is located on the east and north sides of the building

OFFICE POLICIES: (These are very important!)

INSURANCE - We are on the panels of most Managed Care plans. Contact your insurance carrier or our office for assistance in verifying information about your insurance coverage.

CANCELLATIONS - In order to service the needs of our patients, who may have to wait 6-8 weeks for an appointment, it is important that all appointments are attended. Please consider traffic congestion and plan on extra travel time accordingly. New patients should expect to arrive 1/2 hour before the scheduled visit for paperwork and intake forms to be completed. Missed appointments or late cancellations (within 2 business days) are subject to full charges and are due from the patient.
